Ultra orphan diseaseへの取り組み

最近では、orphanならぬ、ultra orphanが注目を浴びているようです。一般に、アメリカでは患者数が20万人以下の疾患をorphan diseaseと呼びますが、特に1万人以下の疾患をultra orphanと呼ぶようになったようです*1。

Ultra orphanに対する治療薬を開発する上での問題点はこのように考えられています。

• A limited pool of patients on which to perform clinical trials;

• Shorter life-spans of patients, because many of the untreated diseases lead to premature death; and

• Limited knowledge of the natural progression of the disease, due to the lack of funding for basic research in, for example, lysosomal storage diseases, such as Hunter syndrome and Fabry disease.

しかし、これらの問題を乗り越えることで、big pharmaと競合しないニッチを手に入れることができ、実際にそれに成功しているのがShireという会社です。この会社の特徴は

とあり、この中でHGTの領域のlysosome病は患者数が1/10万人というくらいに少なく、アメリカでも患者数は五千人程度であったと記憶しています。

Shireをbig pharmaと差別化している要因は、技術的なノウハウ、営業のノウハウ、市場規模の小ささ (年間売上$150-350 million ）などであると考察されています。もちろん、これらの治療薬の開発にorphan drug actが重要な役割を果たしていることは言うまでもありません。

いわゆるOrphan diseaseのエリアはある程度big pharmaが乗り出してくる可能性が高くなると思いますし、現実にそうなってきています。今後はその中でも研究が進んでいない領域や、今回消化したultra orphanの世界にどう切り込んでいくか？がこの業界の大きな課題だと思います。これはいわゆるpersonalized medicineという流れにも一致するところだと思いますし、ここで勝機を見出せねば生き残れない気がしています。これが患者さんと企業とのwin-winの関係につながっていけばよいと思います。
